When someone has a complex illness such as a heart defect (CHD), they often have a long history of procedures, medications, and providers. Families often end up bearing the responsibility of organizing and communicating a health history. These tools can help families to keep track of their health history, and to share relevant information easily and accurately. They can reduce the burden on families, give families confidence in new providers, and increase the quality of care a patient receives.
